I've recently started doing encrypted backups with iTunes before updating my 
iPhone.

It shuts down VoiceOver but you can triple click home to start it
up again and set up the device. When you use the restore from
backup option everything gets restored except for actual apps
which gets downloaded and installed  after the first reboot after
setup. Data for all apps will be restored though.

I'm using Windows 7 with NVDA. I just plug in my iPhone, launch iTunes 
and then tab over to the message that says there's a new update for my 
phone and I tap the space bar on the download and update option. I then 
sit back and wait. Occasionally I'll tab around a bit just to hear the 
updates scroll by. Note, you may need to select the iPhone button to get 
the status of the software on your iPhone.
