To my knowledge, this is a bug in audio ducking. Specifically, if you have 
disabled audio ducking, VoiceOver is very quiet during phone calls. If you 
enable it, the volume is supposed to go back to normal. I haven't personally 
tested this.

VoiceOver Low During Telephone Conversations

I'm quite sure this topic has been discussed in previous posts to this list, 
but a friend has been complaining that VoiceOver on her iPhone 6 is too low 
during conversations.  Can anyone advise the steps to take to remedy this 
situation?
