This happened on my iPhone 6+, beginning in around IOS 10.1 or so, as well as I 
can remember. I thought it was specific to my device, but since then, I’ve 
gotten an iPhone 7, and it does the same thing.

 

Thankfully, it’s easily remedied. Dumping VoiceOver and restarting it always 
puts things right, but I haven’t seen anything about this on the list, and it’s 
hard for me to believe I’m the only one having this happen.

Yes, I have just had it happen once, and as you said, when I turned VoiceOver 
off and back on, it cleared it up. Don’t know what that’s all about. I use my 
air pods most of the time, so it might happen more frequently if I were to use 
corded ear buds.
