Hi scott,
The problem was that the maid for iphone button was still visable. So that when 
I wanted to toggle VoiceOver, when I wanted to turn VO back on, both the Vo 
tripple click as well as the MFI button were both present and once VO was 
switched off, I was unable to toggle it back back on. Now, only the VO button 
is present on the tripple press of the home button instead of both buttons. So 
now, VO always has fockus when toggleing VO back on.

Hi Danny.
If it doesn't go away, you will still be able to pair new MFi hearing
aids in the future. i evaluated a pair of Oticon and then ReSound. I
was able to pair the ReSound hearing aids even though the oticons were
still listed after being removed.
I'm not sure why they advised you to reset everything, since you can
easily dump things out of the accessibility shortcut by visiting
settings, accessibility, accessibility shortcut. Unless I'm missing
something?

No, it wasn't installed using bluetooth settings. It was installed through the 
Settings, Accessibility, Hearing devices section the MFI section. We did check 
the bluetooth settings just to make sure it wasn't lurking there.
