I think I have mine fixed where no one can get through to me, if it is on Do 
Not Disturb, because I never use that function unless maybe in church or for 
some kind of business appointment that wouldn't last more than an hour. Since I 
can't get in my car and go pick up some stranded loved one, I think that on the 
fairly rare occasions, when I put my phone on Do Not Disturb, it does not hurt 
for me to block all calls from coming in. That would be the case with Airplane 
Mode.

sleep mode which puts do not disturb on when the sleep mode goes on.

Just go into your son's contact card and tap on Edit in the top right 
corner, then tap on his ringtone and at the top is an option called "Emergency 
Bypass", turn this on and he will always get through.

A birthday list in a third app would only transfer if the app backs up either 
to iCloud or their own servers.

Exactly, whatever the "Calls From" button says is what is selected and the 
options are Everyone, No One, Favorites or All Contacts.
Then also check if repeated calls is turned on or off, I recommend to have it 
off because if you allow either specific people or your Favorites there is not 
much reason to have this one turned on and scammers often get wise to these 
tricks since they are not stupid.

It won't show as selected while you're in that small group of choices. However, 
after you back out of the "allow calls from" setting, look in the "people" 
dialogue again. After the "also allow", you'll find "calls from". It will say 
"favourites" if that one is selected. Otherwise, it would say something else 
like "everyone".

I don't know what the default is, but you should just go into whichever focus 
you use and check under the People setting, towards the bottom it has something 
called "Calls From" and if it says "Favourites button then Favourites are 
allowed, you can tap on this and review what is selected and also if repeated 
calls is on or not.

You can still allow Favourites and with the new and expanded options for the 
different focus modes you can also specifically allow certain contacts.
When you adjust the Do Not Disturb focus, you have the following 
headings/sections:

Allowed Notifications
Under this heading you can add people and apps, e.g. if you add your son you 
would get notifications from him, this would include calls, messages etc.
Also, in the People section you now find the option to allow calls from 
Favourites and repeated calls, just swipe until you see "Also Allow" and then 
after that "Calls from, Favourites button", tap on this and  you get the 
following choices:

Everyone
Noone
Favourites
All Contacts

Below that is the "Repeated Calls" option which you can turn on or off, the 
explanation is "A second call from the same person within 3 minutes will not be 
silenced".
With respects to allowing apps you would get notifications from whichever app 
you add to the list of allowed apps, e.g. if you had an alarm system you might 
want to allow the app for your alarm system to get through any of the focus 
modes.
You can also decide to allow or not allow notifications which are marked as 
time sensitive.
If you swipe right again there is actually a description for this and it says:
Allow calls and notifications from people, apps and those marked as time 
sensitive if this focus is on.

The next heading is "Options" and here you can turn on whether you want people 
to get a message that you have focus turned on which is called "Focus Status"
You can also set options for what happens on the home screen and lock screen.
If you go into Home screen you can turn on an option which ides notification 
badges while the focus is on and you can even select to hide certain home 
screen pages while it is on. I guess if somebody used their phone for work and 
personal use and maybe they have work related apps on a certain home screen or 
home screens and all their personal apps on another home screen or screens, 
then they could customize the work focus so that during work hours all the home 
screen pages with personal apps are hidden.
The lock screen options allow you to dim the lock screen when the focus is 
active and you can also silence notifications on the lock screen if it is 
active which means you would still get notifications if your phone is unlocked, 
but not when it is locked or on the lock screen.

It won't show as selected while you're in that small group of choices.
However, after you back out of the "allow calls from" setting, look in the
"people" dialogue again. After the "also allow", you'll find "calls from".
It will say "favourites" if that one is selected. Otherwise, it would say
something else like "everyone".

I was under the assumption anyone in your favorites could get a hold of you no 
matter what. Am I wrong about that? If so I’m gonna have to end up doing the 
same thing. Although I have had two people in my favorites that have been able 
to contact me. However I did not set up do not disturb, I set up the sleep app 
sleep mode which puts do not disturb on when the sleep mode goes on.

then tap on his ringtone and at the top is an option called "Emergency Bypass", 
turn this on and he will always get through.

Hi all.  I thought I had do not disturb figured out, but my son, who is on my 
favorites couldn’t reach me last night.  I want to set up a repeating do not 
disturb from 10 PM to 8 AM every day, but allow calls from favorites.  I Can’t 
tell if I’ve done it right or not.  Do not disturb is announced every evening, 
so that part’s right, but I don’t think favorites can get through.   Any help 
would be appreciated.  Thanks.
