No, it doesn't matter if the item already has the 
focus or not. Also, it can be a desktop icon, or it 
can be anything else that requires a double-click. 
There seams to be one exception:
If I open the mouse settings from the Control Panel, 
I can perform the double-click timing test 
successfully (no 3rd click required); however, 
anything else that needs a double-click requires a 
3rd click to work.
