No, it doesn't matter if the item already has the
focus or not. Also, it can be a desktop icon, or it
can be anything else that requires a double-click.
There seams to be one exception:
If I open the mouse settings from the Control Panel,
I can perform the double-click timing test
successfully (no 3rd click required); however,
anything else that needs a double-click requires a
3rd click to work.

What sorts of thing are you clicking on when the
problem occurs?
Are you clicking on things like Explorer shell icons?
I found that if I triple-click, the double-click
occurs.
What may be happening is that the first click causes
a screen update
which causes a delay sufficient to prevent the
double-click being
detected, and that the second and third clicks then
get through
correctly and are processed as a double-click. If
that is the case then
single-clicking on an icon before you actually
double-click it should
work. If that works, we have a potential route to
solving the problem.
