Did you try to run 'php maintenance/update.php' from the command line? If you
can, try this. If you successfully do this and you're *still* not getting the
tables created, then we've got a bug.
If not, you can put the ConfirmAccount.sql file through command line mysql or
phpmyadmin or something (being sure to add the $wgDBprefix before the table
names)
Also, what version of MediaWiki and ConfirmAccount are you installing?
