I have put some basic info about requireing the User-Agent header at
https://wiki.toolserver.org/view/User-Agent_policy. This way, there's a place
where can point people for more info. Please add any useful info to that page.

Thanks, but FWIW, the very first sentence:

Wikimedia sites require a HTTP User-Agent header for all requests.

is false.  (As near as I can tell, the header is required only
for those requests that include an action= modifier.)

as title says I need a extension for user do a checklist for some check item
and save in database or show to others

just like this format:

item1[mark lable or button]

item3[mark lable or button]
item4[mark lable or button]

when user submit and the result can show in result page

but I dont know how to use it in the Mediawiki because the extension is for
Wikiversity only
