Announcement of NFB Karnataka Excellence Awards 2018
Undersigned is glad to inform you that NFB Karnataka as ever invites
applications from visually impaired individuals for “NFB Karnataka Excellence
Award 2018” in different areas
1. The categories of NFB Karnataka Excellence Award 2018 include: -
a. Employee (1),
b. Self Employed (1)
c. Arts/Culture/Sports (1)
d. Social Service (1)
Description of awards in above mentioned categories :-
A. NFB Excellency Award in above mentioned 4 categories is intended for such
visually impaired person of the state who has made extraordinary performance in
their respective filed.
The award in each category carries cash or gifts of worth 10,000/- and
citation
Definition of award in each category
a. Employ – means award to a V.I. Employee either working in public or
private sector for his/her exemplary position/performance and achievements in
professional carrier.
b. Self Employed – means a V.I. person who is self employed in area of
services/trading/industry etc. and achieved an exemplary success in that
particular profession.
c. Art, Culture & Sports – means award to a V.I. Person for his/her
extraordinary performance and exemplary achievements at National or
International level in area of Arts, culture & sports.
d. Social Service – means award to a V.I. person for his/her long and
remarkable voluntary service in any area of human life.
General Rules and procedure for application and nomination to the above
mentioned awards:-
1.-- All the awards are intended only for Karnataka residing visually impaired
individuals
2.—The nomination for each award should be applied on prescribed application
which can be obtain either from office of NFB Karnataka at above mentioned
address or download link of website of NFB Karnataka website:
www.nfbkarnataka.org
3. The application should be accompanied with self attested Xerox copies of
disability certificate, photo identity card, address proof, age proof, two pass
port size photographs, certificates of performance and achievements secured in
respective filed and descriptive resume containing all personnel details
including the type, degree, cause and age of occurrence of visual impairment,
academic qualification including professional skills, detail of the
professional carrier and its success, interests/hobbies, and mission and vision
of life.
4.--Application with all necessary documents must reach to the office of the
federation at above given address on and before 30th November 2018, the
application received after the prescribed date shall not be entertained.
5.--A selection committee under chairmanship of the president of NFB Karnataka
will scrutinize all the applications and take the final decision to nominate
the awardees for all above mentioned awards. Decision of the committee in this
regard will be final and binding to all.
6. The member of the selection committee shall also be entitled to recommend
the name of the nominee for each award, in case if the sufficient applications
are not received for that particular award.
7. The section committee also reserves the right to defers any of the above
mentioned awards without assigning any reason.
8. A common application form for all awards is enclosed with this notice as
annexure -1
9. --The awards will be presented to the nominees on the occasion of annual
function of the federation positively on 4th January 2019 at Bangalore.
10.--Qualifying travel expense of the awardees and their escorts from outside
Bangalore will be reimbursed.
11.--Appropriate lodging and boarding arrangement for awardees and their escort
from out of Bangalore shall be made by the federation
