Thanks David, that was the problem.
It seems when we updated the .net client lib to v16 there were some
breaking changes and setting the ClientCustomerId in the AdWordsUser.Config
dropped out during our refactor.

Hi Kevin,
Your request is using a login that is tied to an MCC account and you are
not targeting (through the clientCustomerId header) a client account.
Campaigns can't be added to MCCs.
