I found several buttons in different toolbars showing the same problem in Impress. I am using OO 2.0. For example in the toolbar "Drawing", I could not include any button neither from the "Visible Buttons" context menu nor from the "Customize" menu. The same happens with many buttons, there is some reaction in the toolbar. It seems the button is added and immediately after removed, althoug it is hard to tell if it is the right button or something else because the change is too fast. Any help on this will be greatly appreciated. If I can help in anyway...
