I tried to save a file I was editing. OO asked me if I wanted to keep my file
as file.sxw or update it to file.odt. I clicked No in the dialog box to tell
OO that I wanted the file changed to the new .odt format. It did so without
checking if file.odt existed. As a result, an existing file.odt was overwritten
without warning.

Apparently I had converted to the .odt format earlier. In this session I
mistakenly opened the older .sxw file.
