There is an old issue report 58124 which relates to this problem.

Just installed the latest version on a Win 7 machine for a client.  Set up a
letter head to be used as the default template, right clicked to set it as
default according to instructions.  Within a session, it comes up whenever
Writer is opened.  However, after the machine is rebooted, it goes back to the
standard blank template.  

Any thoughts?
