I checked with 2.0 (1.9.m125) German version WIN XP: [680m125(Build8947)]:

If you open the attachment for issue 56199 (pls see URL), you will see that
headings with level "heading 2" are not listed in the TOC on page 2. For
example, the heading on page 6 "Management report" (which is visible in WORD) is
not listed.

I checked with 1.1.4 (German) WIN XP: [645m52 (Build 8824)], and there the TOC
looked fine.

I am rather sure that I read something concerning such heading 2 import problems
in another issue, but currently I can't find such issue.
