Choose to create your address book source. Put in a couple of addresses. Continue on to the creation of the address block. There you will see that the street address and ZIP code are left out of your address block, and there is no way to fix it from within this screen, because the address block section is looking for "Address Line 1" and "ZIP", while your address list has "Street" and "ZIP Code". This also occurs if you import the address book over the Wizard (at least coming from a Spreadsheet).
