I've been using is for years and it works good. When you change parameters
manually, you risk causing problems between Server.app and your manual
configuration, which will make the Server.app look unreliable.

Manage these parameters via the Server.app application. It works fine this way.
If you go in the "Mail" section and enable junk mail filtering, it enables
Amavisd.

It also was possible to switch from sendmail to postfix as default MTA since at
least RHEL3.

Yes, it won't be deleted. This is created by spamassassin when it auto-learns
mail it scanned.
I'd recommend you to move this data to a MySQL database.
