I believe the decision is made by the ISS crew.  This website tracks the

I believe that most of the time the station works in digipeater mode.  I
have been experimenting with using this over the past couple of weeks and
its really kinda neat.  It only requires a single-band radio, a fixed
antenna, and a TNC.  http://home.comcast.net/~k9jkm/ARISS_Packet_How_To/ has
great instructions on getting started.

AO-27 and SO-50 offer full-time FM repeaters.
