If the push fields actions are causing table scans, why not index fields
referenced in the push fields if qualification queries? 4.5 million records
is not so much of a big deal?

There is one more criteria that you missed out on..
The view name cannot be longer than 30 characters. So if the form name is 31
characters or more the view name is truncated to 30 characters. This obviously

Never had that problem and I use Firefox personally as well. But I did have
other issues.. For e.g. on Firefox the column names in the table fields are
somehow suppressed from being viewed and you can barely see them if you
hover your mouse above the column title bar in the table fields.. I've
