I think the duplicate field name would be possible only when we update field
meta data from the database directly bypassing AR APIs. Only a guess, but
archgid is one tool that updates field-ids directly in database, so some
scenario could have caused it to mis-behave causing inconsistenty that
I think it means that for your company, the change management configuration
requires that Risk questions are completed before a change is created. Either
this can be disabled ( which may not be right) or you might update SRD to use a
change template that has the risk questions taken care of.

I would suggest that you check if there is any thread deadlock. You can use
jvisualvm ( under jdk/bin dir) or yourkit profiler ( need license).
Easier will be to take just a thread dump to see where each thread is.
