Hello Listers,

  After some testing, I see that all survey related work-flow objects are
attached to
  Request form not helpdesk form. From my customers request, they need to
send
  out survey both requests from request management and incidents in
helpdesk created
  by helpdesk staffs. Since requests would be forwarded to HPD:Help Desk
form.

  Does anyone have any ideas how it can be built?
