I do want to point out that this is a Level(3) issue, not (just) a
Junction issue. So switching providers to any Level(3) reseller will
give you the same problem.

I'm looking for a IAX capable provider with DIDs in
Egypt, South Africa, Nigeria, US and within the EU. Provider should be
used to working with connections over VSAT (they are just so much fun).

This isn't correct. If you read the document closely you'll see that
it says that 'interconnected VoIP providers must offer E9-1-1
