I have been experiencing a similar problem.
I have not yet been able to figure out
what the exact problem is but I know that the errors are inconsitant.
Sometimes nothing for 2 days and sometimes
5 times a day.
I thought about it a lot and I have found

I have setup a2billing to be used as more
of a billing software.
I have made a card for each of my
customers and the call through and get authenticated with callerID and then the
call is placed using dnid.
This is working great.
