Im able to do it through the dialplan by doing this:
Dial(ZAP/1/*67wwww${EXTEN},60)
The 'w' pauses for (I believe) 0.5 seconds - enough time to get the second
dialtone.
Keep in mind that some phone companies charge extra for this.

It would have to be done through who ever provides your POTS service. They
provide the caller ID to who you are calling. Some have the option to
block it. Asterisk cannot be configured to do this.
