In last files I used for testing I had a `\usepackage{amsmath}' line
and this causes `reftex-default-label-alist-entries' to have a
`amsmath' entry, and that's the "problem".

That's the culprit! Indeed, without that entry in the alist the
`form' variable in `reftex-reference'
