The only thing I find difficult to use is the MIDI mixer. Configuration is very much accessible. Although if you try out the new version 2 you may have to OCR the window to get to the different tabs.

Hi.I think I came across this virtual midi synth yesterday but I didn't try it yet. But it sounds the easiest solution out of all. I just hope its settings are accessible with nvda.

I don't know of any USB sound cardes, but have you tried the Coolsoft virtual MIDISynth? That one is pretty easy to set up and all you need to do is get some different soundfonts after configuring it.
