Maybe Telstra have 3 paths between major exchanges and they only report if it
impacts enough individual services to matter but being 3m down I’d hazard a
guess it was a long duct out east, not many customer facing splices.

Surely you're making the assumption that "diverse" equals 2?
Without knowing their network it's impossible to conclude that the loss of
a single path will have any meaningful impact on redundancy.

The damaged duct is owned by Telstra, AAPT aren’t the only ones in it haha.

Not just TPG, was a Telstra cut 3m down which was caught by a bore dipping new
sewer lines
The damage is extensive as the bore pulled the fibre for a significant distance.
