[Axapta-Knowledge-Village] Upgrade 2.5 to 3.0

I'm looking for some information on how to upgrade AX 2.5 to 3.0 sp4.
In perticular, I would appreciate if you could provide me with some advice on:
 
- the upgrade procedure
- what problems should be anticipated and how they should  be solved
