It is the wife that died on 23 Sep 1965 in São Bartolomeu. Following
Cheri's advice, I found
The one on the right is Doc. N°. 4. There's a superscript there. Numero 4.
Just "N" is "nasceu."
It's not clear to me who died. I'd check the baptism of both of them and
see if it was written there as well. Maybe someone else will have a better
take on this.

Can you write things out and not abbreviate? "N." means nasceu or born in
Portuguese. I'm not sure what you mean.
You are in the marriage book, not the death book. In the left column, on
the left page, it is recording the names of the bride and groom. Since you
did not provide the
Marriage N.4 February 27, 1905.
Can't make out the obit on the left column. Is it referring to the bride or
groom? is someone able to translate the obit insert for me?
