Attempting to read these records is not easy but this one is very clear.
We must pay attention to the "articles, de, do e da" these help qualify the
nouns and adjectives so we can properly interpret what we're looking at.
Note it says: do Doutro Caetano Jose de Mesquita. The article do in this

I lean toward Doctor if he had meant “doutro” the letter D should not have
been capitalized. The way to tell for sure would be to check other records in
the time frame and see if his name is written as Doctor.
