php_mapscript has to be use with php as cgi, as you said. But it may happens
that during installation something went wrong somehow, so just in case, if
you see something like extension php_mapscript.so in the php.ini, remove or
comment it.

automatically. Indeed you have to create it manually, as you did.
Concerning the warning yuu see during --install, are you using cartoweb 3.2
?
The warning should not appear after the first --install.

you have an map.ini.in in server_conf/map/ and create the map.ini. Try
to rename you map.ini file to map.ini.in and run again the script
