1) Works fine on the first call from CUCM and CONTINUES to work fine IF
I actually push a key on the CUCM phone so that BACD can route the call
to the appropriate hunt list.
2) Works fine on the first call from CUCM, but FAILS to work on
subsequent call

Yes, you will miss out. Set up a PSTN cloud in your home lab and set up a
frame relay cloud, too. Money well spent, believe me.
