Did you
1. Create MWI ephone-dn
2. dial-peer for mwi
3. mwi application enabled in cue
4. mwi trigger enabled in cue

what happens when you dial the mwi ephone-dn directly? Can you turn it
off and on. If yes, is your cue ntp synching with the ntp master,
becasue  i have seen issue where mwi is not delivered because of
failed ntp sync.
