I managed to provoke this behavior by forgetting to include
'rbd_children' in the Ceph auth setup for the images and volumes
keyrings (https://ceph.com/docs/master/rbd/rbd-openstack/). Doing a:
$ ceph auth list
should reveal if all is well there.

Hi,
some time ago I build small Openstack cluster with Ceph as main/only
storage backend. I managed to get all parts working (removing/adding
volumes works in cinder/glance/nova).
I get no errors in logs but I've noticed that after deleting an
instance (booted from image) I get leftover RBD
