I had some trouble with this tag until I added 'version="2.1"' as a
parameter. Then it worked fine. Sorry I don't have a good address either,
and the documentation seems to be rather worthless.
When trying to get it to work, I found debugging to be helpful:

I had to pull some files off an old CD and I found zip files for the Allaire
CFX_CyberCash tag AND the VTM for the tag. The 2 are in separate zip files
and are about 716 KB.
