Hmmaybe the DLL needs to be registered? Try using regsvr.

I had this ages on a CF5 server...we had to reinstall the whole thing - just
install over the top of the your current install...it should repair it.

we did have to log into the server as admin to finally be able to copy over
the dll.
but the server is still throwing the error.

ok still having the problem.
did a test on the mail server and it is fine.
ran the patch on the server & still having problerms...
