this is a known bug with several versions of IOS 12.1. the answer is to upgrade - or never issue the command 12.1.5Tx seems to be stable. 12.2.12a seems to be OK when I ran into this a year or so ago I had to upgrade from 12.1.2 to something like 12.1.10.
