I configure it this way:
CPPFLAGS=-I/usr/local/src/sendmail-8.12.11/include ./configure
--prefix=/usr/local/clamav --enable-milter
It goes just fine.

Tom, you've probably tried using Stuffit to extract the archive.
Try tar -xzf sendmail.8.13.1.tar.gz
This should extract it properly.
