I have collectd installed on CentOS 5.6 via EPEL repo and have a lot of it
working. I do not seem to be getting Apache stats though.

Resolved by RTFM closer ... needed ?auto at the end of the URL.

After I notice no stats are collected, I start up collectd 5.1.0 with the
syslog plugin enabled and I get the following message in the log (on the
agent), but nothing further and no stat

Hum, I think server can be in v5.x and clients in v4.x but you can't have a
server
I don't think you can use two logs, but I am not positive on this. You are
using syslog and a logfile with that config. I am not sure which one load, or
if either does.
What is in your /tmp/collectd.log?
Do you get any errors when you configure or make?

As I was going in and adding more hosts to our collectd server, I noticed that
some of the hosts I added didn't show up.
I added two new clients which report memcached data and another two which
report apache data. Now I cannot get collectd to stay alive.
