OK, it took many attempts but finally I reproduced this with nvidia.
Turns out it's not a window stacking problem at all. It is compiz
crashing somewhere in the nvidia driver...

Sounds similar to my problem, whenever I select some text or an image and try
to grab & drag it, the cursor teleports to the top left of my screen, when the
unity bar greys out.
Gedit, Firefox, Chromium etc. doesn't matter what application.

After restarting compiz/unity again, it works now. Lots of other things
are broken (Ctrl+Alt+T, Windows+Up/Down, etc.), but at least this
unnerving thing seems to have been an one-time issue.

Another thing I noticed that perhaps is related: sometimes when you call for
the "spread" desktop (super+w) one or all the windows got positioned off
screen. This seems to happen when you have a window slightly off-screen on an
adjacent desktop.
I'm still trying to find steps to reproduce it.
