My results:
* nvidia driver 331.38 + native compiz 0.9.11 = no bug
* nvidia driver 340.24 + native compiz + high performance mode + no flip = no
bug
* nvidia driver 340.24 + native compiz + adaptive mode + fl
I have the same problem with ubuntu 12.04. It's extremely frustrating.

I have completely same problem when using Compiz in Ubuntu classic. Also
the window title sometimes look like inactive even if it has focus and
the titlebar of previous active window appears like active. I've
attached screenshot of this behaviour.
