Apple charges for the major releases, such as 10.4, 10.5. The minor
updates are a free download.
Apple's major OS X major releases have been pretty good since 10.3; most
of the problems have been relatively minor.

Patches are free. You will not be charged again intil X.6, which probably
won't be for 18 to 24 months.
