I guess the thing most people here said is true: everyone has it's on way of "starting". Try different approaches and figure out the one that suits you best.
I always do "it" like that: 1. project in my favorite graphics editor 2. xhtml structure (with almost whole content in) 3. scripts (js, php+mysql) 4. css + graphics from point 1 5. playing with points 2-4 till i get the needed result.
