i can reproduce it too with iceweasel 5 on debian testing.
Switching kde settings - application appearance - gtk+ styles - widget style
from Qt to QtCurve made the problem go away.

Yes, I'm using "usb-audio" hooked up to a Soundblaster Extigy.
I am able to play back with other devices, and I can record with it
