Same problem here. It mostly occurs if there was heavy IO load. It mostly occurs on machine with more than a few CPU's. Its so far only occurred for me on LSI RAID controllers (92xx), with dual quad core Xeon CPU's. Running 'sync before lvremove appears to make it occur less frequently.
