My failure seems similar.  When I start polypaudio, I get the following
in syslog:

Dec 21 14:50:36 localhost polypaudio[8565]: pid.c: stale PID file,
overwriting.
Dec 21 14:50:36 localhost polypaudio[8565]: sound-file.c: Failed to open
file /usr/share/sounds/generic.wav
Dec 21 14:50:36 localhost polypaudio[8565]: Failed to load sound file.
Dec 21 14:50:36 localhost polypaudio[8565]: module-alsa-sink.c: Error
opening PCM device default
Dec 21 14:50:36 localhost polypaudio[8565]: module.c: Failed to load
module "module-alsa-sink" (argument: "sink_name=output"): initialization
failed.
Dec 21 14:50:36 localhost polypaudio[8565]: main.c : Fatal error.
Default sink name (output) does not exist in name register.

It has sometimes worked, but never consistently.

This is an x86 machine, with a 2.6.13 kernel.
