After 3 months, I finally figured it out. enabling url rewrite enables
the "Block hotlinked items" options. The list of approved referers
includes only the *hostname* of the gallery by default.My server
config runs on a nonstandard port, so when I added hostname:port to the
list, it worked.

Using the rewrite module seems to break things. No images within albums show
up with this module enabled. And with the download url on, no images on the
main page show up either. And, deactivating this module isn't enough to fix
the problem.
