when I have a 2.6.x kernel (for example 2.6.14) installed, and add a 2.6.x.y kernel (for example 2.6.14.3), update-grub prefers 2.6.14 and puts it first. Hence, the update is usually not performed if one doesn't manually edit /boot/grub/menu.lst.
