Doing [[TODO]] (while having a TODO.mdwn in place) doesn't render as a link (i.e. it appears as ?TODO). I guess if you have a file named todo.mdwn, then it would link to it. Anyway I think it should appear on the documentation at least. I haven't read all the documentation, but I searched for 'case insensitive' in ikiwiki.kitenet.net with no results, though.
