Try disabling Eaccelerator and report if those crashes happen as well.
Most crashes I have seen were related to Eaccelerator.

EAccelerator disabled, but apache still crashes.

Ok, now try disabling those two nonstandard modules. (Keep eaccel
disabled). My next suspect would be memcache since it does store
objects in memory and that segfault is in memory management.

Hmmm, ok, looks like you have found bug in PHP4 :-).
Are you able to test same configuration with unstable version of php4
4.4.4? F.E. in chroot?

Build php 4.4.4-1 and apache2_2.0.55-4.2 from source with additional
debugging information.
Lots of Segmentation faults in error.log but no core dumps.
Still trying to get one.
